1. 1 copy of reissue application form
(You do not need to enter the nursing care worker training period and exam information)
2. 1 3×4cm photo (photo taken within 6 months from the date of application)
3. 1 copy of ID card (front and back)
4. 2,000 won money order (fee): Cash can be exchanged for a money order at the post office.

5. Address: Contact the social welfare department of the relevant city, county, or district
