② Doctor’s opinion
It must be submitted with the long-term care certification application, but if you are 65 years of age or older, you can submit it before submitting the review materials to the rating committee.
The doctor’s opinion must be submitted within the specified deadline according to the request for issuance of a doctor’s opinion guided by the Corporation after the certification investigation. If it is not submitted, a grade cannot be assessed.
