There is one thing to keep in mind when applying. In case of closure or leave of absence for more than one month, an employment maintenance action plan must be submitted every month, but the standard for ‘monthly’ is different for closure and leave of absence.
Example) When closed or on leave from April 15 to May 31
In case of closure
You must submit an employment maintenance action plan from April 15 to April 30 and an employment maintenance action plan from May 1 to May 31.
In case of leave
You must submit an employment maintenance action plan from April 15th to May 14th and a plan from May 15th to May 31st.

③Individual employee’s consent form for suspension or leave of absence, minutes of labor-management consultation meeting, or labor-management agreement (prepared separately and uploaded when applying on the employment insurance website)
A labor-management agreement is only required at workplaces with 30 or more employees. In addition, the boss can substitute documents negotiated with worker representatives (a person representing the majority of workers) or documents confirming individual worker consultations. There is no specific form. If it is difficult to appoint a separate employee representative or create a labor-management agreement, it is better to obtain a consultation document from individual workers. We will share the form with you so that you can use it. This form has been inspected by a labor attorney, so you can use it with confidence.

④Employment rules, collective agreement, or employment contract (prepare separately and upload when applying on the Employment Insurance website)
If there are no employment rules or collective agreements as data to determine existing working hours, it is replaced with a labor contract that allows individual employees to know their working hours.
⑤Data proving damage due to COVID-19 (prepared separately and uploaded when applying on the Employment Insurance website)
-Sales ledger, value-added tax base certificate, inventory ledger, profit and loss statement, tax invoice, reservation cancellation certificate, business suspension recommendation, documents related to raw material supply and demand, etc.
※ You do not have to submit all the data, but you just need to submit at least one piece of data that can be proven.
Example) If you are scheduled to take leave in May 2019, prove that sales in April 2020 decreased by 15% by comparing sales in April 2019 and April 2020.
